Not having a parental authorization form can be a troublesome problem. It can delay in treatment of care.
For example, if a baby has a serious injury or burn, if we do not have this form, it puts us in a bind. We don't want to be providing treatment to a child that could potentially be harmful for a child. Having this information opens the door and helps us care for this small person, without delay and without any confusion.
