One of the fears a lot of people have is that if someone can hurt themselves so severely that maybe they could turn that anger on other people and hurt them as well. That's actually very rare. Most self-injurers will say that they rather hurt themselves than anyone else, but it's a form of self-sacrifice, and so these folks are generally not a danger to other people.
